Discussion of a student's transition from high school begins early in a student’s educational experience and continues throughout a student’s school career. Beginning no later than the year in which the student turns 16, the IEP team identifies postschool goals and outcomes based on assessment of the student’s strengths, capabilities, interests and preferences and develops a Transition Plan as part of the IEP. Students are invited to attend transition planning meetings and serve as active participants. Representatives of community agencies are invited to attend the meetings to build linkages in anticipation of service needs for the student as an adult. Annual transition goals/benchmarks for community employment, vocational training, post-secondary education, independent living, and community participation are written (as appropriate) in the annual Post Secondary Transition Plan of the IEP.
